Material Charge Waiver

Release of Information - Use this form to allow student account information to be released to someone other than the student or the parent on record. This form should be presented to our office in person by the student. If it is mailed or faxed to our office, it MUST be notarized. Only information pertaining to your student account will be released.

Student to Student Grant fee waiver form - Is a refundable fee, assessed during the fall and spring semesters, used to provide support for needy and disadvantage students. Refund application must be made in the Student Accounts Office by the 15th day of the semester you are requesting (See the Billing Information Brochure for specific dates and/or our web site under the Billing Information section )
